At this time, we do have a waiting list. We are working diligently to serve everyone in the order in which we receive the Personal Information Form.

We appreciate your patience and understanding.

APPOINTMENT INSTRUCTIONS:

Before we schedule your first appointment, we need you to complete and submit the Personal Information Form and PLEASE READ through the Consent Form.

Once we receive the Personal Information Form, a member of our staff will contact you to schedule your first counseling session.

If you are requesting marriage counseling, each spouse will need to submit a separate Personal Information Form.
